Registered nurses require extensive training and are essential to the practice of home health care. They are certified to dispense medication, monitor vital signs, and evaluate health and medical needs. They build a case management model that:

Physical Therapists
Many patients recovering from an injury or illness truly benefit from the use of physical therapy.
Should your Plan of Care or Physician determine that one would be beneficial, the RN Case
Manager will refer one for an initial visit. Physical therapy has been proven to be significantly
effective in easing pain and improving function and mobility.

After providing a complete examination, the therapist will be able to decide which treatment,
if any, would be productive in our client’s recovery. By incorporating these treatment plans into daily living, the affect should be quite obvious.
Physical therapists use a variety of treatment strategies. They may employ different types of exercise, which not only promote strength, but muscle endurance as well. Some may even use less routine therapies like electrical stimulation or ultrasound technology.
Whether the problem is age, injury, or disease related, physical therapy is always a viable solution.

Licensed Vocational Nurses & Certified Home Health Aides
Licensed Vocational Nurses, LVNs, are state-licensed skilled nurses providing skilled nursing services to patients, as per the Plan of Care, under the supervision of a Registered Nurse. LVNs are able to perform additional services if mapped out in the Plan of Care by the supervising RN. LVNs may also take on a supervisory role, aiding in the management of Certified Home Health Aides. LVN duties may include:

Requirements: Licensed by the State’s Board of Nursing as a LVN, excellent communication skills are required to build effective working relationships with the customer and their families. Organizational skills as well as dedication to seniors are preferred qualifications.

Requirements: Certified by the State’s Board of Nursing as a Certified Home Health Aide, excellent communication skills are required to build effective working relationships with the customer and their families. Organizational skills as well as dedication to seniors are preferred qualifications.

State-certified, the Home Health Aide has been trained to provide hands-on, personal care providing physical and emotional support to patients in their homes under the direction of a RN or LVN. Additional related services such as light house keeping or laundry may also be required depending on the Plan of Care and individual case parameters, as outlined by the registered nurse.
